This level of care ensures a safe and supportive environment so you receive the assistance you need, without disrupting fellow residents, to maintain your wellbeing and quality of life.

Understanding High Care Support in Aged Care
High Care Support in a residential aged care setting involves a comprehensive approach to address acute and chronic health and care needs of individuals using a holistic approach to address cultural concerns within a resident community.
The level of care required can vary significantly among individuals. When considering the admission of a new resident, a Residential Aged Care home must consider both the needs of the new applicant and the current resident community to ensure a sustainable level of expert care.

Standard Emergency Response:
Emergency Call Systems are located in all of our rooms, which are easily accessible to residents for immediate assistance. We have clearly defined plans for various emergency scenarios that are communicated to you and your family upon admission and during our regular orientation sessions.
Highly trained nursing staff:
Adequate resourcing of highly trained Registered Nurses is critical so we can comprehensively assess, plan and implement complex care needs as they evolve. High Care Support often involves two-person assist which requires additional resourcing pressures.
Monitoring equipment:
It may be necessary to continuously monitor your vital signs, including blood pressure, heart rate, respiratory rate, and temperature. This requires additional Vital Sign Monitors. You may require tools to monitor your blood glucose levels regularly if you live with diabetes.
Mobility, Safety and Comfort:
We may need to ensure we have adequate space for Mechanical Lifts to transfer you if you have limited mobility in addition to easy access to walking aids. If you need to use a wheelchair or comfort chair, we have this equipment in all our homes. You may need to pay for your own equipment if you need it for your sole use. We can help you procure your own and guide you through your funded options.
All our homes have King Single electric beds which require room to operate effectively. Please note, lifting equipment and resources for bariatric care is limited.
Pressure Relieving Mattresses may be necessary to prevent and manage pressure ulcers.
Respiratory Support:
If you have a respiratory condition requiring supplemental oxygen, we’ll need to ensure you have access to Oxygen Therapy Equipment. For residents with standard respiratory disorders, we’ll need to have adequate Nebulizers and Inhalers.
If you’ve had a tracheostomy procedure, you may require a ventilator. Please note, we do not have the resources or expertise to manage ventilators at any of our homes.
Memory Support:
People living with significant cognitive impairment may require a dedicated memory support unit. Our ability to offer a range of assistive technologies and dedicated care relies on a suitable ratio of staff and bed availability at each of our homes. We’ll evaluate our capacity at the time of your application and on a case by case basis.
Medication and Wound Management:
You may require our assistance with the accurate and timely administration of your medications (Medication Administration Devices). This may require Medication Management Software, a system to track and manage your medication schedules and doses.
Various types of wounds require specialised dressings for optimal care.
Nutritional Support:
In some acute cases, Feeding Tubes and Pumps may be required for residents with difficulty swallowing or maintaining adequate nutrition orally.
We cater to individual dietary needs and restrictions through Specialised Diet Programs.
Mental Health Support:
We have a standard Lifestyle Program with activities to promote social engagement and emotional wellbeing.
In certain cases, we may need to recruit the support from our Allied Health partners such as Counselling Services if you need to access a mental health professional for emotional and psychological support.
Communication Aids:
We may need to implement our Communication Boards to assist you if you have speech difficulties.
Infection Control:
As standard, all employees are trained in Standard Precautions and we have extensive supplies of Personal Protective Equipment (PPE): Including masks, gloves, and gowns for infection prevention.
Hand Hygiene Stations are located around the facilities to promote frequent handwashing and infection control practices.

Before we meet with you:
We will review your ACAT assessment report, which outlines your health and care needs. If we’re confident we can meet your recommended level of care, we will arrange to meet with you in person. This process includes our evaluation of the specific services and facilities available at the home you are applying, to ensure we have the adequate staffing resources, amenities, equipment and suitable resident culture to ensure you fit like a glove.
We come to you:
Wherever you are when you apply to live at one of our homes; whether you’re still living at home or about to be discharged from hospital, we do our best to come to you for your personal assessment (where reasonably possible). We like to call this our mobile pre-admission clinic.
Care planning:
Collaborating with you and your family, we review your preferences and goals. We’ll discuss aspects of your lifestyle like daily routines, social activities, dietary preferences, and any specific cultural or religious considerations.
After we meet:
We will contact you and your family to discuss the outcome of the assessment, the suitability of our home, and any specific considerations. We hope to be able to welcome you to our family, in which case we will guide you through the admission process, including paperwork and funding considerations. In the case we’re unable to currently meet your unique needs, we will place you on a waiting list and assist you to find a suitable solution.

We understand that each individual's care needs are unique, and in instances where our services may not fully meet specific requirements, seeking additional support is crucial.

Allied Health Professionals:
Consulting with allied health professionals, such as physiotherapists or occupational therapists, can provide additional insights into specific care requirements.
If you live in the Macedon Ranges you may be eligible for low cost community health services, nutrition groups, dietetics, diabetes education and free counselling at Macedon Ranges Health.
Specialised Health Facilities:
For individuals with complex medical needs, exploring specialist care or hospitals may be beneficial.
